HEALTH SERVICES MESSAGE

Welcome to the Jay School Corporation Health Services! Our goal is to promote and protect the health of our students because children are “The Health of Our Future”. The School Nurse strives to achieve and maintain the best possible health status of all students so they may benefit fully from educational experiences.

The Nursing Staff is there to care for your ill or injured child, but we also focus on providing health education and health promotion. We present classroom instruction throughout the school year, such as giving developmentally geared talks about Hand Washing and Hygiene to grades Pre – 5 at the beginning of the school year; Dental Care in February during National Dental Health Month for grades K-2, and a "Growing Up" Puberty presentation to fifth grade boys and girls. TAR WARS, a smoking prevention program, is presented each Spring to fourth graders that teaches about the harmful effects of tobacco. The high school age student also learns about topics related to self care. We love bringing healthy subjects into the classroom!

DISEASES

Each new school year, the State of Indiana requires schools to give information out to parents regarding specific contagious diseases. Information about HPV, Meningitis and Pertussis can be accessed below.

CONCUSSIONS & SUDDEN CARDIAC ARREST

The following information is provided about Concussions due to a new Indiana State Law that went into effect July 1st, 2012.
The Law states that Schools must inform and educate high school coaches, student athletes and parents of student athletes on the nature and risk of concussion and head injury to student athletes, including the risks of continuing to play after a concussion or head injury.
Each year, before beginning practice for an interscholastic or intramural sport, a high school athlete and parent must be given concussion information, sign an acknowledgment receipt form and return the form to the coach. The athlete will not be permitted to participate until the signed form is on file.

An athlete suspected of sustaining a concussion must be removed from play, even during a practice or game. The student must be evaluated and receive a written clearance to return to play by a licensed Health Care Provider trained in the evaluation and management of concussions and head injuries.

MEDICATION QUICK FACTS

  • All medications given at school must have a note from the parent/guardian with date, student’s name, name of the medication, amount to be given, when it is to be given, reason for medication and length of time the medication will be given.

  • Medications should be given at home whenever possible. Once a day, twice a day and even three times a day medications can usually be managed at home.

  • Non-prescription medications (those bought over-the-counter) must be in the original package/container, not a baggie, envelope, etc. Over-the-counter medications will only be given according to the directions on the package, unless the school receives a doctor’s order to do otherwise. The school will need a signed note from the parent/guardian with date, student’s name, name of medication, amount to be given, when it is to be given, reason for medication and length of time the medication will be given.

  • Prescription medications must be in original container with pharmacy label or written instructions from the doctor. Send to school (in the original container) only what is needed for the doses to be taken at school. Prescription medications will only be given according to doctor’s written instructions or the instructions on the pharmacy label. The school will need a signed note from the parent/guardian with date, student’s name, name of medication, amount to be given, when it is to be given, reason for medication and length of time the medication will be given. Changes in long-term daily medication must have a written note from the doctor.

  • Parents are responsible for assuring medication arrives safely at school.

  • Medication will not be sent home with a student eighth grade or younger. Ninth thru twelfth grade students may bring medications home if the school has received written permission from a parent/guardian. (2001 Indiana State Law)

  • After contacting parent/guardian regarding pick-up, or the conclusion of the school year, all medications not picked up within a timely manner will be destroyed.
